In response to the recent announcement by the Department of Health to create 'Academic Health Science Networks (AHSNs)', the North East and North Cumbria have submitted an application to become an AHSN.
‘A Good Death’ refers to the creation of a positive End of Life experience for people in their last years of life through the provision of community based care and support.
This project aims to pilot an approach which will promote ‘A Good Death’, in a person’s place of choice, as a fundamental part of the care that clients receive.
Home Group will develop integrated care pathways across the whole continuum of care with the objective of ensuring that clients receive the right support, in the right place and by the right person.
As part of the objective of providing a holistic approach to care, Home Group will ensure that support staff respond with compassion and empathy and develop a coordinated approach with other support services in the care of terminally ill clients.
Home Group are working collaboratively with Marie Curie Cancer Care, Public Health North East, Age UK, and Newcastle University.
